Anthem Blue Cross Dental Net I was referred to an oral surgeon by my primary care dentist since I have to have 6 teeth and 2 residual root tips extracted in advance of getting a full upper denture. All the extractions including the root tips have to be done during one visit, which necessitates the use of IV sedation. My general dentist cannot perform any of this work. Anthem Blue Cross Dental Net denied the original pre-authorization request, and subsequent appeal stating that my general dentist can perform the services being referred. Which is absolutely not the case. I even spoke with a customer service rep who indicated that the extraction of residual root tips is definitely a referable service and it clearly states that in their Dental Net Provider Contract. She has no explanation for why the appeal was denied. My primary care dentist has even provided Anthem Blue Cross Dental Net with his specific reasons for referring this work to an oral surgeon and I still can't get anyone at Anthem Blue Cross to re-evaluate the original denial and the appeal and authorize the necessary work. I have filed a complaint with the California Department of Insurance, although I don't hold out much hope for any results from that effort. HELP... what are my options? Who can I contact? Thanks

I was a passenger in my friend's car when he was ticketed for having an open container of alcohol in the backseat. This occurred in Illinois, and the text of the relevant law is at the bottom. He was ticketed for violating section A; I was ticketed for section B. I confirmed with the very nice lady at the DMV that no points will be placed on my license since I was the passenger, but she was unsure as to what kind of report will be sent to the Secretary of State's office to be added to my driving record. So, here's my question: since I'm not getting any points for this, is it likely to affect my insurance rates? I can see both possibilities - on the non-affect side, it's a non-moving violation, but on the yes-affect side, it involves alcohol (albeit peripherally in my specific situation). How much on average would liability insurance cost after 2 dwi's (5 years ago)?

It's been over 5 years (and sober!). I'm on a tight budget though and need a car for work now. I already figured what the interlock will cost me, but I have no idea on insurance. I don't know if it will be $100-$300 a month. After the first DWI (bad college years), it was about $125 a month I think. So I'm really hoping it stays under $250 at least a month just for liability. I will be driving at least 30 miles per day on week days and about 12 miles per day on the weekend. I am 18 and in college. I haven't gotten my driver's license but plan to take the test in a month or two. I don't know what the MPG of my car will be as I have not purchase it yet but I can almost guarantee it will be fairly old so that may help determine it somewhat. An estimate on a cheap car would be nice too.

Okay, so I'm a 17 year old girl in michigan. I am planning to take my road test soon and then get my license. I have divorced parents and my grandfather on my moms side is going to buy me a car and keep it in his name. He is going to put me as the primary driver on this car and pay for the insurance. However; I live with my dad and therefore my address on my license will be at my dad's house, since you have to have proof of residency when you go to secretary of state. My dad has been extremely annoying about this situation, saying that his car insurance will go up since I live here, so I need to know if it will or not. The people I have talked to so far say that it won't since I am not a driver on his car or his wife's. I've been wanting to get my license so I can get a job and whatnot for over a year now, so please help me with your experience. Thank You :)
